What is the Adult Placement Scheme?

Wrexham Adult Placement Scheme is a registered service within the Adult Social Care Department. The aim of the service is to provide a short break in a "Home from Home" by inviting someone to share and take part in every day family life. The person will be 18 years and over who may have a Learning Disability, Mental Health needs, a Physical, Neurological and/or Sensory Impairment or someone who is Older and physically frail.

What is involved?

As an Adult Placement Carer you will be inviting someone to share and take part in your every day family life.  By doing this you will be providing a "Home from Home" service which will enable the person to become part of and participate in family life and link into the existing social networks you as a family enjoy.

By providing an Adult Placement Short Break you will also be offering much needed support to a family carer enabling them to continue to care for as long as possible.

The length of the Adult Placement stay will be negotiated between the Adult Placement carer, the person and their family and will be based on the outcome of a needs assessment.

Payment will be based on the Adult Placement Fee level.

Prior experience is not essential but you must be able to demonstrate a commitment to supporting people to maximise their independence enabling people to remain in their own home for as long as possible.  You will receive ongoing training and support to ensure that you have the skills you need to do this.

You will be working with vulnerable adults and an enhanced Criminal Records Bureau check will be undertaken.
