If your baby was born on or after 29th October 2002, please ensure that you take your hospital discharge notes with you to the Register office when visiting to register your baby's birth. These discharge notes contain the baby's NHS number and enable the Registrar to issue a form for you to register the baby with your family doctor.
Should you forget to take the notes with you, don't worry, as your family doctor will be able to obtain the NHS number from a different source.
Application forms available in 'Downloads' area.
Copies of Birth, Marriage and Death Certificates are available from the Register Office. These are needed for many day-to-day purposes including:
- Applying for a passport
- Seeking a student loan
- Attaching to a CV
- Making an insurance claim
- Proving probate
For each copy certificate you need to let us know the:
- Name of the person (and the name of their partner in the case of a marriage)
- Date of the event
- Place of birth or death
- Name of the church for a marriage
