Organisations providing support schemes as part of the Housing Support Grant (HSG).  

The HSG is an early intervention grant programme, funded by the Welsh Government. The programme has been set up to help some of Wales’ most vulnerable people to live independently in their own home or supported housing.

Adferiad

Scheme: Wrexham Housing

Service description 

Three shared living properties that support occupants with their daily living skills to promote and encourage independent living.

Two of the properties (5-bedroom and 6-bedroom) provide support to occupants from 9am-8pm daily. The third property is 3-bedroom shared living accommodation for adults with lower-level needs.  

There is also a self-contained one-bedroom flat.

Scheme: Wrexham Housing (24-hour)

Service description

One 5-bedroom shared living property offering 24-hour staffed support including waking night staff and a sleep-in staff member.

The service provides support with daily living skills to promote and encourage independent living.

Service description

Support with contacting mental health services, GP, benefits, debt. People moving from temporary accommodation to independent/ supported living are supported to develop their tenancy-related skills and to move on to other support services.

Scheme: Outreach / Floating Support

  • Eligibility: People with mental health ill health who have been referred by mental health professionals.

Service description

Outreach support for individuals with mental health support needs within the community.

This can be used as a preventative measure to remain within their own home and local community, or step-down support for individuals moving from supported living to live independently. This support is flexible to the individual’s needs and preferences.

Scheme: Supported Flats Scheme

  • Eligibility: People with mental health ill health who have been referred by mental health professionals.
  • Scheme capacity: 6

Service description

This supported accommodation scheme is for people who are recovering from mental ill health and who are ready for a move to more independent living. They could be homeless following a stay in hospital, or could have been living in more intensively-supported accommodation.

Up to 10 hours of support per week will be provided by staff based within the scheme, at times to suit the individual.

The accommodation provided is self-contained one-bedroom flats. The support covers life skills, budgeting, household skills and social skills, helping prepare people for independence.
