Use this form to supply information about a non dependant’s earnings. This form can be used in conjunction with claims for Second Adult Rebate.

Use this form to supply the Council with information about your self employed earnings if you do not have sufficient accounts to prove your earnings.

Earnings Certificate

Earnings Certificate (Applicant and Partner Only)

Use this form to supply the Council with proof of your earnings if you haven’t got adequate payslips available.

Housing & Council Tax Benefit Appeals

Request for reconsideration or appeal.

If you think our decision is wrong, you can ask us to:

  • Look at the decision again
  • Pass your case to an independent tribunal run by the Appeals Service.

To request for either of the above, please write to us explaining why you think our decision is wrong. We must receive your letter within one month of the decision we made.
