	Application for Housing Benefit, Council Tax Benefit and Second Adult Rebate Please use this form to claim benefit for the first time or following a break in your claim.
	Housing Benefit, Council Tax Benefit and Second Adult Rebate Review Form This form will usually be sent to you by ourselves if we decide to review your claim. This form should only be downloaded and completed if your original form was damaged or lost in the post.
	Housing and Council Tax Benefit Change in Circumstance Form For Pensionable Age claimants only Use this form to notify the Council of any changes in your circumstances, including a change of address. Please use this form to tell the Council of your new address and the date when you moved.
	Housing and Council Tax Benefit Change in Circumstance Form For Working Age claimants only Use this form to notify the Council of any changes in your circumstances, including a change of address. Please use this form to tell the Council of your new address and the date when you moved.
	Review Form This form will usually be posted to you if we decide to review your claim. Please do not return this form to us as we will collect it from you. This form should only be downloaded and completed, if your original form was damaged or lost in the post.
	Certificate of Rent Paid To be completed by Landlords only Please get your landlord to complete this form if you do not have a tenancy agreement.
	Income Certificate For non householders only Use this form to supply information about a non dependant’s earnings. This form can be used in conjunction with claims for Second Adult Rebate.
	Self Employed Earnings Information Use this form to supply the Council with information about your self employed earnings if you do not have sufficient accounts to prove your earnings.
	Earnings Certificate (Applicant and Partner Only) Use this form to supply the Council with proof of your earnings if you haven’t got adequate payslips available.
	Housing & Council Tax Benefit Appeals Request for reconsideration or appeal. If you think our decision is wrong, you can ask us to: Look at the decision again Pass your case to an independent tribunal run by the Appeals Service. To request for either of the above, please write to us explaining why you think our decision is wrong. We must receive your letter within one month of the decision we made.
	Housing Benefit BACS Payment Form (Claimant) Please use this form to inform of your new payment arrangements.
	Housing Benefit BACS Payment Form (Landlord) Please use this form to inform of your Landlord’s new payment arrangements.
	Direct Payment to Landlord Form This form needs to be completed by your landlord before we can make payments of Housing Benefit directly to them.
	Statement Sheet Please use this form to inform the Council of any miscellaneous information that you cannot easily convey by the use of the forms shown on this page.
